Please fill out session paperwork before your first appointment. You can also download and print this form and bring it to your session if that is easier for you.

Fees and Cost of Sessions

The fee is $160 for a 45-minute session. A reduced fee is available based on need.

What Insurance Plans Cover Session (or part of session) Fees?

• Out-of-Network Coverage . If you can choose your own therapist or provider find out your coverage now learn more.

If you have out of network benefits, the insurance company will reimburse you directly. Therefore, you pay the session fee of $160 and we will provide you with a receipt for each session to submit to your insurance for reimbursement (Note: reimbursement will depend on your coverage, deductible, etc.).

What You Need to Bring to Your First Appointment

Please complete this session paperwork before your first appointment. All you need to bring to your sessions is the fee (this will depend on your coverage, dedictible or other fee if you are not using insurance).

Session Billing and Payment Information

We accept credit cards for all session fees (we add an 3.5% fee for each session to cover credit card billing expenses). We can reduce the session fee in some circumstances.

Due to the high cost of billing, all payment and co-pays are due at each session.

Cancellation Policy

Kindly give 24 hours notice when cancelling an apppointment or a $100 charge is made for time reserved. Services are by appointment only. We reserve an appointment time exclusively for each person. The 24 hours cancellation notice allows us to offer that time to someone else in need of counseling. Maplewood Counseling Services appreciates your cooperation.
